As a "Doctor's Only" company it is our national policy to open accounts for health care professionals that fit a limited criteria, which is as follows:

A health care professional who has graduated with a four-year degree from an institution that has been accredited by the U.S. Department of Education.

A health care professional who has passed the state exams and/or has been licensed in their discipline by their state.

If you meet this criteria, click on the link below to get our New Client Information Form.  Complete it and email or fax  it along with a copy of your license or degree and we will begin the process of establishing an account for you.
